REVIEW OF THE COMPLETENESS OF FILLING OUT THE INPATIENT INFORMED CONSENT SHEET IN THE PEDIATRIC HEALTH SCIENCE MEDICAL STAFF GROUP (KSM) TO SUPPORT THE QUALITY OF SERVICE AT DR. HASAN SADIKIN BANDUNG HOSPITAL

Based on the preliminary study on 15 February 2020 in RSUP Dr. Hasan Sadikin Bandung found incomplete inpatient medical record file, especially on the form Informed consent According to secondary data obtained by the author that in December 2019 with the amount of 75 Informed consent, average-filling 49 complete and 26 incomplete with a percentage of completeness (65%) and (35%) Incomplete. Thus it can be said that charging Informed consent has not run optimally. The purpose of this research is to know the completeness of filling Informed consent in the medical staff group (KSM) Children's Health sciences at RSUP Dr. Hasan Sadikin Bandung.
The method of research used is a descriptive method with a quantitative approach. Data collection techniques are observations, interviews and literature studies. Research instruments using Stationery, interview guidelines and form cheklist.
The results of the research show the completeness of filling Informed consent form which is researched by researchers in January and February 2020 is 96 Informed consent. The amount of Informed consent in january is 48 with completeness (82%), incompleteness (18%). The number Informed consent in february Is 48 Informed consent with completeness (76%), incompleteness (24%). Overall the percentage of completeness (79%), incompleteness (21%), accurate (79%), inaccurate (21%). According to the standards based on regulation of the Minister of Health RI No. 129/Menkes/SK/II/ 2008 about minimum Hospital service standard Filling form Informed consent is 100%. The advice provided should improve the cooperation of the officers responsible for filling Informed consent especially in the medical staff group (KSM) Children's Health sciences so that the problem does not happen again.
